Diagnostic Testing for Canine Lyme Disease: A New Two-Tiered Approach for Effective Management

ثبت نشده
چکیده

Limits of the Traditional Approach The traditional approach is limited because these tests measure antibodies to the whole Borrelia burgdorferi organism (called wholecell antibodies). Whole-cell antibodies elevate after exposure or vaccination, and often remain elevated after successful treatment. Traditionally, dogs are only tested after clinical signs are evident. The lack of early clinical signs, such as erythema migrans, in infected dogs places greater importance on the laboratory method used when diagnosing canine Lyme disease. With recent research showing that subclinical infections of canine Lyme disease are more prevalent than once thought, the diagnostic approach to these cases should be reexamined.

Collection and characterization of samples for establishment of a serum repository for lyme disease diagnostic test development and evaluation.

Serological assays and a two-tiered test algorithm are recommended for laboratory confirmation of Lyme disease. In the United States, the sensitivity of two-tiered testing using commercially available serology-based assays is dependent on the stage of infection and ranges from 30% in the early localized disease stage to near 100% in late-stage disease. Other variables, including subjectivity in...

A concise critical analysis of serologic testing for the diagnosis of lyme disease.

Diagnostic testing for Lyme disease in the clinical setting primarily relies on assessment of serologic responses to infection, with the exception of the early localized phase of disease, in which the diagnosis must be made clinically, due to the recognized insensitivity of serologic testing at this phase of disease. For the diagnosis of early disseminated and late disease, the Centers for Dise...

Development of a Multiantigen Panel for Improved Detection of Borrelia burgdorferi Infection in Early Lyme Disease.

The current standard for laboratory diagnosis of Lyme disease in the United States is serologic detection of antibodies against Borrelia burgdorferi. The Centers for Disease Control and Prevention recommends a two-tiered testing algorithm; however, this scheme has limited sensitivity for detecting early Lyme disease. Thus, there is a need to improve diagnostics for Lyme disease at the early sta...

Lyme disease testing by large commercial laboratories in the United States.

BACKGROUND Laboratory testing is helpful when evaluating patients with suspected Lyme disease (LD). A 2-tiered antibody testing approach is recommended, but single-tier and nonvalidated tests are also used. We conducted a survey of large commercial laboratories in the United States to assess laboratory practices. We used these data to estimate the cost of testing and number of infections among ...

Poor Positive Predictive Value of Lyme Disease Serologic Testing in an Area of Low Disease Incidence.

BACKGROUND Lyme disease is diagnosed by 2-tiered serologic testing in patients with a compatible clinical illness, but the significance of positive test results in low-prevalence regions has not been investigated. METHODS We reviewed the medical records of patients who tested positive for Lyme disease with standardized 2-tiered serologic testing between 2005 and 2010 at a single hospital syst...
